We provide Health Care Attendant services to individuals living in Manhattan the a 30 mile surrounding radius.
Health Care Attendant: These services enable people who are elderly and/or disabled to live independently. Their quality of life is enhanced without the stress of interrupted routines and changes in daily habits. It also provides companionship which is as important as health care when it enables people to remain independent and in their homes.

Home Health: Home is more than a place to stay, it’s a favorite chair, a beloved pet, a treasured memory, someone you love. However, sometimes illness, a chronic condition or a disability, can make living at home difficult.
Our Home Health services have the solution to these problems.
Home Health care can be the perfect match for many individuals as they recover from an illness or injury in the comfort of their own home. It helps them regain their independence and become as self-sufficient as possible.
Home Health is provided by health care professionals such as registered nurses, licensed medical social workers, registered physical, occupational and speech therapists, certified home health aides and a registered dietician.
The Home Health staff provide and help coordinate the care and/or therapy the person’s doctor orders. Along with the doctor, Home Health staff create a plan of care to reach and maintain the person’s best physical, mental, and social well being. Once the person’s home health goals are achieved they can return to their active, independent life.
Hospice: Hospice is a specialized, compassionate concept of care that recognizes hope as a powerful force that sustains life throughout a serious illness.
An early referral to Hospice is of great benefit to the client, caregivers and family members. The client and family may discuss Hospice care at any time with their health care professional - doctor, nurse, hospital staff, etc. Or meet with Homecare & Hospice staff at any time to discuss hospice. Hospice is an appropriate choice as a treatment option for management of pain and physical symptoms, as well as social, emotional and spiritual concerns. The individual’s physician prescribes Hospice care and continues to be involved as part of the treatment team.
The Hospice team cares for clients in their homes, nursing homes, assisted living centers at The Good Shepherd Hospice House. They help clients to be as comfortable as possible through expert symptom management, supporting them and their families, providing respite to the caregivers and other practical assistance.
The team includes physicians, nurses, social workers, chaplains, certified home health aides and volunteers who receive extensive training in caring for people with terminal illnesses.
Levels of care at The Good Shepherd Hospice House:
General Inpatient Care: a level of care made available to all hospice client’s who are in need of pain control or symptom management that cannot be provided in any other setting. GIP IS considered short term care because the team’s job and overall goal is to get the client’s acute symptoms to a controlled level within 24-48 hours.
Respite Care: is short term inpatient care provided to a client to relieve the family members or other persons caring for the individual at home. Medicare and most insurance pay to give the caregiver a break from the 24-hour task of caring for a loved one. Respite care allows the client’s caregiver up to five (5) consecutive days “off” from taking care of the client.
Residential Care: provides hospice clients a place to live that provides 24 hour care. A hospice client may move to the Hospice House simply because of the client/family’s desire to reside in the Hospice House or it may relate to caregiver or safety issues.

Home Health and in-home Hospice care is for individuals living in Manhattan (including assisted living centers or nursing homes) and a 30 mile surrounding radius. Hospice clients cared for at The Good Shepherd Hospice House do not have to live within that service area as the hospice house will accept clients residing in any county.
Health Care Attendant services do not require a doctor’s order and anyone can refer to services. We provide Health Care Attendat services to individuals living in Manhattan and a 30 mile surrounding radius.
